Vaccine Clinic for Age 12 and Over

Rite Aid, in cooperation with the City of Plymouth, is offering a Pfizer Covid First Vaccination Clinic at the Plymouth Cultural Center at 525 Farmer on May 18 and 19 from 10 a.m. to 7 p.m. for everyone age 12 and over. If you receive a first shot at this clinic, you will be scheduled for your second shot on Wednesday, June 9 (regardless of whether you attended the May 18 or May 19 clinic).

Those who are age 16 or 17 must have a consent form signed by a parent or guardian prior to receiving their vaccine. Those age 12-15 must have a consent form signed by a parent or guardian and be accompanied by an adult.

There is no charge for vaccine, but adults who attend will be required to present photo ID and, if they have it, an insurance card. Those who do not have an insurance card will still be able to obtain the vaccine without charge.

This clinic will be held concurrently with the second vaccine clinic for those who received shots on April 27 or 28. Those with second appointments should come at the same time as their first appointment and bring their vaccine card and the signed consent form.
